A Certified Death Certificate is necessary before anything can be done. A death certificate can be obtained through a Funeral Director. It is a good idea to obtain multiple copies of a death certificate as most agencies require a certified certificate and not a photocopy. This is what the funeral director needs for the certificate.

- Armed Service (years, branch, Service # copy of DD214)
- Race
- Hispanic orgin (if yes specify)
- Number years of schooling
- Social Security number
- Martial status
- Surviving spouse (maiden name if wife)
- Usual occupation
- Kind of business
- Name of business
- Residence of decedent
- Father's name
- Mother's name
